It’s almost time!  Back to School for our FLS Family is quickly approaching!  We are so excited to have everyone back together for another great year.

Every year, FLS gathers a lot of forms and information from our families to help us get the school year started with the most up to date information on our students.  All Back to School forms and Information for our families can be found on this webpage.  They will also be sent directly to you via email.

For forms that needs to be returned to the school, please print each form, review, and complete the information requested.  Once the forms are completed, please send the completed forms to the school office.  

• If sending the forms via email, please send to flsoffice@flsweb.org
• If mailing the forms, our address is 526 NW 4th Street  Faribault, MN 55021
• Or, you can simply drop the forms off at the school office.  In order to avoid large amounts of people in our office at this time, we ask that you place your forms in an envelope, and drop them in the secure mailbox on the west side of the building (across from Walgreens).

Title I Form

Important, it is required that all families complete the Application for Educational Benefits – Free and Reduced Meals.

FLS offers healthy meals each day.  Children in grades K-8 may buy lunch for $3.15.  Breakfast is also available for students in grades K-8.  There is no cost for Kindergarten breakfast.  For children in grades 1-8, the cost is $1.80 per meal.

Your child(ren) may qualify for free and reduced-price school meals.

Title III Form

Minnesota is home to speakers of more than 100 different languages. The ability to speak and understand multiple languages is valued.

The information you provide will be used by the school district to see if your student is multilingual. In Minnesota, students who are multilingual may qualify for a Multilingual Seal upon further assessment.

Additionally, the information you provide will determine if your student should take an English proficiency test. Based upon the results of the test, your student may be entitled to English language development instruction.

Access to instruction is required by federal and state law. As a parent or guardian, you have the right to decline English Learner instruction at any time.

Every enrolling student must be provided with the Minnesota Language Survey during enrollment. Information requested on this form is important to us to be able to serve your student.

Student, Staff, and Family Community Contract COVID-19

The COVID-19 global pandemic makes this an unprecedented time. For us to be together, in-person at our school, we must commit to ways of learning, and working that actively promote the health and safety of everyone: students, staff, and members of the greater community. This is a commitment like no other.

The Student, Staff, and Family Community Contract applies to all students and staff who participate in in-person instruction (academic and/or athletic) on our campus in 2020-2021. All students and staff must adhere to our new policies and procedures.

Snack Cart

Every day, the students at FLS enjoy a break time around mid-morning. We understand that it’s hard to remember to pack a snack everyday for your child(ren) so we offer the snack cart program.

For a low fee each semester, you child can pick a snack from the cart, to enjoy during break time. It’s one more way we hope to make our families lives a little less stressful!

There is a lot of information that is involved with Faribault Lutheran School and don’t expect you to remember it all!
